How is a stress fracture diagnosed?

Short answer

A stress fracture is assessed through your activity history, precise tenderness and loading tests, and imaging when a bone injury needs confirmation.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

What the clinician needs to know

Tell the clinician about recent changes in running, walking, jumping, work demands, footwear, or training surface. Pain that began with activity and became easier to trigger is relevant, as is pain in the foot, heel, shin, or thigh. The examination maps tenderness and checks walking, swelling, and whether weight-bearing is safe. An early plain X-ray may not show every stress injury. Depending on the findings, follow-up imaging such as magnetic resonance imaging may be considered. Bone health factors, including Vitamin D Deficiency, may also be discussed.

Protect the area while waiting

Stop impact activity that causes pain and do not test the area by repeatedly hopping or running. Use only the amount of weight-bearing advised by a clinician; a boot or crutches may be appropriate for some injuries. Keep a note of pain at rest and with walking. Do not restart impact exercise until symptoms and clinical guidance support it. If blood tests identify a deficiency, Vitamin D Supplementation should be taken only as directed.

When to seek prompt care

Arrange medical review for focal bone pain that persists, worsens, or causes a limp. Seek urgent care if you cannot bear weight, pain is severe at rest, the limb is visibly swollen or deformed, or symptoms followed a major injury. Sudden severe pain with numbness, a cold foot, or colour change requires immediate assessment.

Questions for the appointment

Ask which bone is suspected, whether you should use a boot or crutches, and when repeat imaging may be useful. Ask how to maintain fitness safely, whether bone-health tests are relevant, and what milestones should come before a gradual return to impact.

Questions people ask

Can a stress fracture be present when an X-ray is normal?

Yes. Early stress injuries may not be visible on a plain X-ray, so the clinician may use follow-up assessment or another scan when suspicion remains.

Should I walk on a painful leg?

Limit weight-bearing until a clinician advises what is safe, particularly if walking causes a limp or sharp focal pain.
