How is a stress fracture diagnosed?

What the clinician needs to know
Tell the clinician about recent changes in running, walking, jumping, work demands, footwear, or training surface. Pain that began with activity and became easier to trigger is relevant, as is pain in the foot, heel, shin, or thigh. The examination maps tenderness and checks walking, swelling, and whether weight-bearing is safe. An early plain X-ray may not show every stress injury. Depending on the findings, follow-up imaging such as magnetic resonance imaging may be considered. Bone health factors, including Vitamin D Deficiency, may also be discussed.
Protect the area while waiting
Stop impact activity that causes pain and do not test the area by repeatedly hopping or running. Use only the amount of weight-bearing advised by a clinician; a boot or crutches may be appropriate for some injuries. Keep a note of pain at rest and with walking. Do not restart impact exercise until symptoms and clinical guidance support it. If blood tests identify a deficiency, Vitamin D Supplementation should be taken only as directed.
When to seek prompt care
Arrange medical review for focal bone pain that persists, worsens, or causes a limp. Seek urgent care if you cannot bear weight, pain is severe at rest, the limb is visibly swollen or deformed, or symptoms followed a major injury. Sudden severe pain with numbness, a cold foot, or colour change requires immediate assessment.
