ER describes a laboratory feature, not the size, grade, or extent of the abnormal area. Those other findings still guide the treatment plan.
How to read the pathology result
DCIS means abnormal cells are inside the milk ducts and have not been seen invading nearby breast tissue. ER-negative means the sampled cells do not show the estrogen receptor that would allow estrogen-blocking medicines to act on them.
The report may also describe grade, margins, necrosis, and the amount of tissue involved. A negative ER result does not by itself mean that DCIS has become invasive, and it should not be confused with a complete receptor profile for any separate invasive cancer.
Planning the next steps
Review the biopsy report and breast imaging with a breast surgeon. Treatment commonly focuses on removing the involved area with clear margins. Depending on its extent and position, the discussion may include breast-conserving surgery or removal of the breast.
After breast-conserving surgery, the team may discuss radiotherapy based on the final pathology and your circumstances. Ask whether receptor testing needs confirmation on the surgical specimen and whether any finding suggests invasive disease. Hormone therapy should only be considered if another hormone-sensitive finding gives it a clear purpose.
When to contact your care team
Arrange a timely review after the diagnosis so that imaging, pathology, and surgical choices can be considered together. Seek prompt clinical advice for a new breast or underarm lump, bloody nipple discharge, skin dimpling, or a newly pulled-in nipple; these changes need assessment but do not establish the cause.
After biopsy or surgery, contact the treating team urgently for worsening redness, increasing swelling, pus, fever, heavy bleeding, chest pain, or difficulty breathing. Follow the discharge instructions supplied for your procedure.

Ask what the report says about grade and margins, how imaging defines the area, which operation fits its location, and whether radiotherapy is likely after breast-conserving surgery. It is also reasonable to ask who reviewed the ER test and whether the final surgical tissue will be tested again.

Is ER-negative DCIS invasive breast cancer?
No. DCIS is defined by abnormal cells remaining inside the ducts. If invasion is found in another specimen, that finding is assessed separately.
Will hormone therapy treat ER-negative DCIS?
Hormone-blocking medicine generally does not target an ER-negative lesion because its cells lack the relevant receptor. Your team may discuss it only if another hormone-sensitive finding provides a reason.
Does ER-negative describe the grade?
No. Receptor status and grade measure different features, so both should be read in the pathology report.
