Dark urine and porphyria

Porphyria describes uncommon disorders involving the way the body makes haem, a component used in blood proteins. Some forms cause urine to become reddish, brown, or darker after exposure to light or air. Episodes may also involve severe abdominal pain, constipation, vomiting, back or limb pain, tingling, weakness, anxiety, or confusion. Other forms are linked with skin sensitivity to sunlight and blistering.

Dark urine on its own does not establish porphyria. Dehydration, medicines, food pigments, blood, liver conditions, and muscle injury can produce similar shades. A personal or family history of repeated unexplained attacks can help guide assessment.

Arrange medical review if dark urine recurs, particularly when it follows illness, fasting, a new medicine, or sunlight exposure. Keep a record of urine colour, timing, pain, bowel symptoms, neurological changes, skin reactions, and possible triggers. Do not start restrictive diets or stop medicines without professional guidance. Testing may involve urine, blood, or stool samples during or near an episode, followed by specialist advice if indicated.

Seek urgent help for severe abdominal pain with vomiting or confusion, new weakness, trouble breathing, seizures, fainting, or rapidly worsening neurological symptoms. Get prompt assessment for yellow eyes, fever, very little urine, or dark urine after strenuous exercise.
