Dandruff Flakes Under a Microscope: What Should You Know?

A flake by itself is not a complete diagnosis. A clinician may examine scale, hair or a skin sample when the appearance is atypical or an infection is being considered. The assessment also includes where the scale occurs, whether it is greasy or dry, and whether there is redness, itch, broken skin or hair loss. Ordinary dandruff and seborrheic dermatitis are often recognised clinically rather than by examining a flake alone.

Do not deliberately scratch until the scalp bleeds or apply harsh chemicals to create a sample. Bring a list of shampoos, oils, dyes and styling products, including anything recently changed. Follow the clinician's instructions if a sample is collected. Depending on the findings, care may include a medicated shampoo treatment or topical antifungal treatment, with directions matched to the diagnosis.

Arrange an assessment when scaling is thick, one-sided, painful, oozing, associated with patchy hair loss or unresponsive to sensible care. Prompt attention is appropriate for rapidly worsening redness, swelling, pus or fever. These findings can require examination of the scalp rather than reassurance based on the appearance of flakes alone.
