Cyst on the back of the neck: what to know

A skin cyst is a sac beneath the skin that may contain keratin. On the back of the neck it can catch on collars, hair, helmets, or bedding. A quiet cyst may be skin-coloured and mobile, while inflammation can make it tender, red, warm, or swollen. A small opening or discharge may occur, but not every neck lump is a cyst, and a visual check cannot establish the diagnosis.

A clinician will consider how long it has been present, whether it changes, and whether it moves with the skin. These details help decide whether observation or a procedure is appropriate.

Avoid squeezing, lancing, or repeatedly pressing the lump. Keep the area clean and reduce rubbing from collars or straps. If it is uncomfortable, arrange a dermatology review rather than waiting for repeated flare-ups. When a cyst keeps returning or causes ongoing irritation, cyst removal may be discussed; the clinician will explain the procedure, wound care, and whether timing should wait until inflammation settles.

Arrange prompt care if the neck becomes increasingly painful, red, hot, swollen, or starts draining pus. Fever, chills, spreading redness, or feeling unwell raises concern for infection. Seek assessment for a lump that is hard, fixed, ulcerated, bleeding, rapidly changing, or associated with difficulty moving the neck or swallowing.
