What surgery may involve

A hysterectomy may be performed through an abdominal incision, through the vagina, or with keyhole instruments. The uterus is removed; the cervix may or may not be removed. The fallopian tubes and ovaries are separate decisions. Removing the ovaries causes an immediate loss of ovarian hormone production, while leaving them may allow hormone production to continue until natural menopause.

Doctors may recommend hysterectomy for fibroids, adenomyosis, endometriosis, heavy periods, endometrial hyperplasia, or cancers of the womb or cervix. It is a major decision because pregnancy is no longer possible afterward.

Preparing and recovering

Before surgery, discuss the reason for the operation, alternatives, planned route, and which organs will be removed. Tell the team about medicines, allergies, previous operations, and future fertility wishes. Recovery includes wound care, gradually increasing movement, and avoiding activities for the period your surgeon advises. Ask when driving, work, sex, and lifting can resume.

Arrange help at home and attend follow-up. Emotional responses can vary, especially when fertility or ovarian function changes; support from your care team can help.

When to contact the surgical team

After surgery, contact your team for fever, worsening pain, heavy vaginal bleeding, redness or discharge from a wound, difficulty passing urine, calf swelling, or shortness of breath. Emergency help is needed for severe breathlessness, chest pain, fainting, or sudden deterioration.
