What should you know about croup without fever?

Croup commonly follows a viral infection and may include a barking cough, hoarse voice, or stridor. Fever can be absent, mild, or present, so a normal temperature does not rule out croup and does not show how hard a child is working to breathe. Pay attention to breathing at rest, chest movement, colour, alertness, and drinking. A common cold may precede symptoms, but diagnosis depends on the whole pattern.

Keep the child upright and calm, give fluids in small sips when comfortable, and avoid smoke. Check breathing when the child is settled and keep a clear route to medical help if symptoms change. Do not use hot steam or medicines prescribed for someone else. A clinician can decide whether an examination, medicine, or nebuliser treatment is needed even when there is no fever.

Call emergency services for blue or grey lips, pauses in breathing, severe pulling around the ribs or neck, inability to swallow, or unusual difficulty waking the child. Seek urgent assessment if noisy breathing continues while calm, drinking drops, the child cannot speak or cry normally, or breathing becomes progressively harder. A fever-free episode still warrants prompt review when airway signs are present.

Ask how to distinguish croup from other causes of noisy breathing, what to do if symptoms return overnight, and whether your child needs follow-up. Explain how you measured temperature, describe any cough or hoarseness, and report changes in drinking and urine.
