What should you know about croup without barking cough?

Croup is swelling around the larynx and upper windpipe. If swelling is limited, a child may mainly have hoarseness, a tight-sounding voice, or stridor when upset, without the classic barking cough. Other illnesses can also cause cough or noisy breathing, including a common cold, bronchiolitis, asthma, allergic rhinitis, or reflux. A persistent cough may need a different assessment, such as for chronic cough or acute bronchitis; the sound alone cannot identify the cause.

Keep the child calm and upright, avoid smoke and strong scents, and offer fluids when swallowing is comfortable. Observe the child while resting rather than judging severity during a crying spell. Do not give leftover antibiotics or use hot steam. Record the sound if it comes and goes, and arrange medical advice when hoarseness or noisy breathing is new, recurrent, or not settling.

Seek emergency help for struggling breaths, chest indrawing, blue or grey lips, pauses in breathing, drooling with difficulty swallowing, or unusual sleepiness. Same-day assessment is appropriate when noisy inhalation occurs while calm, the child is drinking poorly, or symptoms are worsening. A cough-free presentation does not remove the need for urgent care if the airway appears narrowed.

Ask whether the sound is coming from the throat or chest, what other causes should be considered, and which changes require emergency action. Mention exposure to colds, allergies, reflux symptoms, previous breathing episodes, and whether the child is comfortable between episodes.
