Croup not getting better: next steps

Short answer
Croup that is not getting better needs reassessment, especially when noisy breathing persists at rest, drinking falls, or breathing takes more effort.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Recheck sooner
- Breathing worsens or noisy breathing occurs at rest
- Support
- Calm, upright positioning and frequent fluids
- Emergency signs
- Blue colour, severe effort, drooling, or reduced responsiveness
Why symptoms may persist
Croup irritates and narrows the upper airway, often alongside a viral cold. The barky cough and hoarse voice may linger, but worsening breathing, reduced alertness, or difficulty drinking should not be explained away as a lingering cough.
A clinician may need to listen to the child, check oxygenation and hydration, and consider another explanation for the symptoms. Describe when the illness began, whether noisy breathing occurs at rest, and whether any treatment has already been given.
What to do next
Keep the child calm and upright, give small drinks often, and avoid smoke. Follow prescribed instructions exactly; do not repeat or borrow medication without advice. Contact a child health consultation service when symptoms are failing to improve, returning frequently, or limiting sleep and fluids.
Take a short note of breathing sounds and triggers if this can be done without delaying care. A clinician may discuss a steroid or nebuliser treatment when airway swelling or breathing difficulty warrants it.
When delay is unsafe
Call emergency services for severe breathing difficulty, ribs or neck visibly pulling in, persistent harsh breathing while the child is settled, blue or grey colouring, pauses in breathing, drooling with trouble swallowing, or unusual difficulty waking the child.
Seek same-day advice if the child is drinking poorly, urinating much less, becoming more tired, or the cough and noisy breathing are clearly worsening. Do not wait for fever before acting; serious airway trouble can be present without it.
What to ask at reassessment
Ask what improvement should look like, whether the diagnosis remains croup, how to recognise stridor at rest, and whether prescribed treatment should be changed. Confirm the correct dose, timing, and follow-up plan for your child's medicines.
