Croup cough and wheezing: what to know

A barking cough and harsh noise when breathing in can reflect narrowing high in the upper airway, as happens with croup. Wheezing is usually a musical sound from narrowed lower airways, often more noticeable when breathing out. A person with asthma, acute bronchitis, bronchiolitis, or COPD may wheeze; allergic rhinitis can contribute to airway irritation. Anaphylaxis can cause sudden breathing difficulty with swelling, hives, vomiting, or faintness and is an emergency. A recording of the sound can help a clinician, but it should never delay urgent care.

Keep the person upright and calm, loosen tight clothing, and move away from smoke or other irritants. Follow an existing clinician-provided reliever plan exactly; do not share inhalers or start someone else’s medicine. Offer fluids if swallowing is easy. Observe whether the sound occurs at rest or only after coughing, and check for chest pulling-in, fast breathing, or difficulty speaking and drinking. A nebuliser treatment is sometimes used by clinicians when medication needs to reach narrowed airways, but home treatment should follow medical advice.

Call emergency services for severe breathlessness, blue or grey lips, faintness, confusion, sudden facial or tongue swelling, or inability to speak in full phrases. Seek urgent assessment for noisy breathing at rest, visible pulling between the ribs, rapidly worsening wheeze, or poor drinking. Children with a new croup-like cough and any breathing difficulty should be assessed promptly.
