Cramps during sex before a period

Deep penetration can press on the cervix or pelvic organs, and pelvic floor muscles may tighten in response to discomfort. Hormonal changes before bleeding can make some people more sensitive. Pain during sex may also occur with endometriosis, adenomyosis, fibroids, an ovarian endometrioma, pelvic inflammatory disease, vaginal dryness, or an infection. The direction matters: cramps are a symptom and do not by themselves show that one condition is present. Pain with bleeding, discharge, fever, urinary symptoms, or bowel movements gives a clinician more information.

Pause or stop sex when pain begins, and try positions that allow control over depth. A slower pace, more arousal, and a suitable lubricant may reduce friction; avoid products that irritate the vagina. Note whether the pain is at the entrance or deep inside, its relation to the cycle, and whether it continues afterwards. Arrange a medical review if the problem repeats, limits intimacy, or occurs with abnormal bleeding or discharge. Do not use hormonal contraception as a self-diagnosis.

Seek urgent care for sudden severe pelvic pain, faintness, heavy bleeding, fever, repeated vomiting, or feeling very unwell. Arrange prompt assessment for pain after sex with unusual discharge, bleeding after sex, pain when urinating, or a possible pregnancy. These signs can need examination and testing rather than watchful waiting.

Tell the clinician whether pain is deep or superficial, whether it follows a cycle, and whether it persists after sex. Ask whether pelvic examination, infection testing, or imaging is appropriate, and discuss options such as the combined pill or hormonal coil only after your goals and health history are reviewed.
