Cramping and pain during sex

Cramping and pain during sex can result from dryness, muscle tension, infection or a pelvic condition and should not be ignored if recurrent. Pain may be at the entrance, deeper in the pelvis, during penetration or afterward. You deserve care that takes the location, timing and effect on intimacy seriously.

Low lubrication, irritation, involuntary pelvic-floor tightening or skin inflammation can make penetration painful. Genitourinary syndrome of menopause may cause dryness and fragile tissues; lichen sclerosus and vulvodynia can affect the vulva. Deep cramping may occur with endometriosis, an ovarian endometrioma or pelvic inflammatory disease. These conditions can overlap, and pain during sex does not identify one cause by itself.

A clinician may ask whether pain is superficial or deep, whether it varies with your cycle, and whether there is discharge, bleeding, urinary discomfort or a history of pregnancy or surgery.

Stop or change activity when pain begins; consent includes being able to pause. Try slower pacing, positions that limit depth and a suitable lubricant if dryness may contribute. Avoid fragranced products that irritate the area. Arrange a confidential appointment if symptoms recur, and ask about pelvic-floor therapy, menopause care or treatment for an identified condition. Do not self-treat suspected infection without advice.

Seek urgent help for severe or sudden pelvic pain, fainting, fever, vomiting, heavy bleeding, or pain with possible pregnancy. Arrange prompt review for new pain with foul-smelling discharge, sores, urinary symptoms, bleeding after sex or pain that is steadily worsening.

Ask what the pain location suggests, whether you need infection testing or an examination, and which lubricants or treatments are appropriate. Ask about pelvic-floor support and whether laparoscopic excision of endometriosis is relevant only if endometriosis is confirmed and other options have been discussed.
