This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

The short answer

A cramp in a stomach muscle is a sudden tightening of the abdominal wall that may follow exertion, coughing or fluid loss. It can feel like a hard knot or brief spasm and may improve when the muscle rests.

What it can mean

An abdominal-wall cramp can follow new exercise, twisting, repeated sit-ups, heavy lifting or a forceful cough. A Muscle Strain may cause soreness that is worse when you move, cough or touch the area. Fluid loss from Dehydration can encourage muscle spasms. Conditions such as Hypothyroidism, Chronic Kidney Disease, Vitamin D Deficiency or Sarcopenia may be relevant when cramps are frequent, but a single spasm does not identify any of them. Pain deeper inside the abdomen may have a different cause from a muscle cramp.

What to do now

Pause the activity and breathe slowly. Keep the abdominal wall relaxed, use a comfortable position and try gentle warmth if the skin sensation is normal. Sip fluids if you may have been sweating or have had vomiting or diarrhoea. Once the spasm has settled, return to movement gradually. Avoid vigorous stretching, deep massage over severe pain and strenuous lifting until the area feels comfortable.

When to seek care

Get prompt medical advice for persistent or worsening abdominal pain, repeated vomiting, fever, a swollen or rigid abdomen, faintness, blood in vomit or stool, or pain after an injury. Seek urgent care if severe pain begins suddenly or you cannot keep fluids down. Recurrent spasms, a visible bulge, or pain with coughing should also be assessed.

Questions for your doctor

Tell your doctor whether the pain is in the abdominal wall or feels deeper, what movement brings it on, and whether bowel, urinary or menstrual symptoms occur. Ask whether examination or testing is needed before restarting exercise.

Questions people ask

How can I tell a muscle cramp from internal abdominal pain?

A muscle spasm is often linked to movement, coughing or touching the abdominal wall. Deeper pain, fever, vomiting or bowel changes need medical assessment.

Can dehydration cause a stomach muscle cramp?

Fluid loss can contribute to muscle spasms. Replace fluids gradually, but seek care if you cannot drink or symptoms are severe.
