Coughing Phlegm With Blood Spots: What to Know

Tiny red flecks can appear after frequent coughing because the lining of the throat or airways becomes fragile. Blood can also drain from a nosebleed or irritated gums and then be coughed out. Phlegm may be clear, white, yellow, or green for several different reasons, so its colour does not diagnose a bacterial infection.

Pay attention to whether the spots are one-off or recurring, whether the blood is bright red, and whether it is mixed into phlegm. A cough with breathlessness, chest pain, fever, night sweats, unexplained weight change, or marked weakness needs more careful evaluation.

Write down when the spots appeared and what you were doing beforehand. Check for nose or gum bleeding, avoid smoke and vaping, and sip water if your throat is dry. Book a medical appointment when spots recur, the cough persists, or you have a condition affecting the lungs or blood clotting. Do not use someone else’s prescription medicine.

Call emergency services if you cough up more than spots, the bleeding does not stop, or you develop trouble breathing, chest pressure, severe dizziness, fainting, confusion, or blue lips. Seek same-day care if you are becoming feverish and very unwell or the blood appears repeatedly. Bring a list of medicines, including blood thinners, to the assessment.

Ask what source of bleeding is most likely after an examination, whether your lungs and oxygen level need checking, and what changes should trigger emergency care. Tell the clinician about recent respiratory illness, allergies, smoking, vaping, travel, injury, and any medicine that affects clotting.
