COPD and right-sided heart failure: what should you know?

When lung disease reduces oxygen exchange or changes the small blood vessels in the lungs, the right side of the heart may need to pump against greater resistance. This can lead to right-sided heart failure, also called cor pulmonale. Possible features include swollen ankles, increasing tiredness, reduced exercise tolerance, abdominal swelling, or a change in breathlessness. These symptoms can also occur during a COPD flare or with another heart or lung problem, so they need assessment rather than self-diagnosis.

Arrange a clinical review if ankle swelling is new, weight is rising quickly, or ordinary activities are becoming harder. A clinician may check your oxygen level, heart and lungs, blood pressure, medicines, and kidney function, and may arrange an ECG, chest imaging, or echocardiogram. Keep prescribed inhalers and heart medicines available, take them as directed, avoid smoking, and ask before changing salt, fluid, or diuretic plans. Seek emergency help for sudden or severe breathlessness, chest pain, fainting, new confusion, blue lips, or a racing or irregular heartbeat with weakness.
