COPD with normal SpO2: understanding the reading

A pulse oximeter estimates oxygen saturation through a sensor, usually placed on a fingertip. The reading can change with activity, rest, illness, cold hands, movement, or a poor sensor fit. It does not measure how hard you are working to breathe, how narrowed the airways are, or how well you tolerate exercise.

Someone with COPD may have a normal reading while resting and still experience cough, wheeze, chest tightness, or breathlessness during exertion. Clinicians combine the reading with symptoms, examination, medical history, and tests such as spirometry.

If you are checking at home, sit quietly, warm your hand, keep still, and allow the device to settle. Record the reading alongside your symptoms and activity rather than relying on one isolated result. Home devices can be inaccurate, so share concerns and repeated unusual readings with a clinician.

Continue prescribed treatment as directed and have your inhaler technique checked during a chronic disease review. If you smoke, support from a smoking cessation programme may help protect lung function. Do not use oxygen or alter medicines solely because of a home reading.

Get urgent help if breathing becomes severe or rapidly worse, you cannot speak normally, you develop blue or grey lips, feel confused or faint, or have severe chest pain. Seek prompt advice for a marked change in cough or phlegm, fever with worsening breathing, or repeated readings that concern you.

Ask what your reading means in relation to your usual level, whether you need spirometry or a chronic disease review, and how to recognise an exacerbation. Ask which symptoms matter more than a single number and when to call for help.
