COPD with normal spirometry: what it can mean

Spirometry measures how much air you move and how quickly you blow it out. Results can be affected by technique, recent illness, medicines, and whether the test captured your usual symptoms. A clinician may review the effort and repeat the test if the result does not fit your history.

Breathlessness, cough, wheeze, or chest tightness can have several explanations. Asthma may vary over time, while bronchiectasis or chronic cough may need different assessment. A normal test is therefore one piece of the clinical picture, not a diagnosis on its own.

Take the report to the clinician who requested it and describe when symptoms occur, such as during exercise, at night, or after exposure to dust or smoke. Mention flare-ups, mucus, previous chest infections, and all inhalers or other medicines.

Depending on your history, the next step may include repeat spirometry, a review of inhaler technique, or another assessment. Do not start, stop, or change inhalers without advice. If you smoke or vape, a smoking cessation programme can support a planned quit attempt.

Seek urgent medical help for severe or rapidly worsening breathlessness, blue or grey lips, new confusion, fainting, or difficulty speaking in full sentences. Arrange prompt clinical review for coughing blood, a new persistent chest pain, or symptoms that are steadily worsening even when the test was normal.
