What should you know about COPD breathing?

Short answer
COPD can make breathing harder by narrowing airways, so assessment, prescribed treatment, and a flare-up plan matter.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Common breathing changes
- Breathlessness, wheeze, chest tightness, cough, or phlegm.
- Assessment
- Spirometry measures airflow.
- Urgent concern
- Severe or rapidly worsening breathlessness needs immediate help.
How COPD affects breathing
With COPD, air may move less freely through narrowed airways and damaged lung tissue. You may notice breathlessness during activity, wheezing, chest tightness, a persistent cough, or phlegm. Breathlessness can vary from day to day and may worsen with infection, smoke, dust, or exertion. These symptoms can have other causes, so a clinician should assess a new or changing pattern.
[Spirometry](/treatments/diagnostics-and-screening/spirometry/) checks how much and how quickly you breathe air out. It is one part of evaluating COPD and planning care; symptoms and examination also matter.
Ways to manage breathing
Use inhalers exactly as prescribed and ask for a demonstration if technique is uncertain. Pace activities, pause before you become exhausted, and sit upright when breathless. Gentle breathing with a longer, relaxed exhalation may reduce the feeling of trapped air for some people. Avoid tobacco smoke and other irritants; a [Smoking Cessation Programme](/treatments/lifestyle-and-wellness/smoking-cessation-programme/) can help if you smoke. A [Chronic Disease Review](/treatments/consultations-and-assessment/chronic-disease-review/) can update your plan.
Breathing warning signs
Call emergency services for severe breathlessness at rest, blue or grey lips, confusion, collapse, or difficulty speaking. Get prompt medical advice if breathing is clearly worse than your usual level, if a reliever is not helping as directed, or if cough, wheeze, phlegm, or fever changes. A sudden change should not be attributed to COPD without assessment.
Questions for your doctor
Ask how to recognise your usual breathing pattern, which inhaler is for quick relief, how to check your technique, and what steps to follow during a flare-up. Ask whether spirometry or a review is needed for new symptoms.
