COPD and pulmonary hypertension: what to know

Short answer
COPD can contribute to pulmonary hypertension, so worsening breathlessness, dizziness, or ankle swelling deserves a focused medical assessment.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

What pulmonary hypertension means
Pulmonary hypertension is raised pressure in the arteries carrying blood from the heart to the lungs. Long-term lung disease can reduce oxygen exchange and change these blood vessels, making the right side of the heart work harder.
Breathlessness during activity, fatigue, dizziness, chest discomfort, a racing heartbeat, or swollen ankles may occur. These symptoms have several possible causes, including COPD itself, so they do not establish pulmonary hypertension on their own.
How it is assessed and managed
Tell your clinician if your walking distance has fallen, you need more rest, or your ankles swell by the end of the day. Assessment may include oxygen measurement, an electrocardiogram, breathing tests, blood tests, and heart imaging.
Use inhalers as prescribed and avoid smoking and inhaled irritants. Keep follow-up appointments so oxygen needs, heart strain, and treatment response can be reviewed. Do not start oxygen or change its flow without specialist instructions.
When symptoms are urgent
Call emergency services for sudden severe breathlessness, fainting, blue lips, severe chest pain, or confusion. Seek prompt medical review for new dizziness on exertion, rapidly increasing ankle swelling, or breathlessness that is markedly different from your usual COPD.
Questions for your doctor
What could explain my change in exercise tolerance? Do I need heart imaging or an oxygen assessment? How should I use oxygen if it is prescribed? Which symptoms suggest right-sided heart strain or an emergency?

Questions people ask
Is pulmonary hypertension the same as ordinary hypertension?
No. Pulmonary hypertension concerns pressure in the lung circulation, whereas ordinary hypertension concerns pressure in the body's main arteries.
Does breathlessness prove I have pulmonary hypertension?
No. COPD, infection, heart conditions, anaemia, and other problems can also cause breathlessness, so testing is needed.
