Constipation and pain during intercourse

A full rectum or straining can increase pressure in the pelvis, making deep penetration uncomfortable. Pain may also relate to vaginal dryness, pelvic floor muscle tension, infection, endometriosis, or another pelvic condition. Irritable bowel syndrome can cause bowel symptoms with pelvic discomfort, but constipation does not explain every episode of pain. The location, depth, timing, and relationship to bowel movements help a clinician narrow the possibilities.

Do not continue intercourse that hurts. Use communication, adequate lubrication, and positions that limit depth if you choose to try again, stopping when pain returns. Treat constipation with fluids, gradual dietary fibre, movement, and a clinician-approved medicine when needed. Arrange a confidential appointment if pain repeats, affects intimacy, or comes with discharge, bleeding, urinary symptoms, or pelvic pain. A pelvic examination is offered with explanation and consent.

Get urgent medical help for sudden severe pelvic or abdominal pain, fainting, heavy bleeding, fever, repeated vomiting, or a rigid or markedly swollen abdomen. Seek prompt assessment for pain after sex with unusual discharge, sores, painful urination, or bleeding, or for constipation with blood in stool or inability to pass gas. These symptoms can need treatment beyond bowel care.
