Congestive heart failure shortness of breath: what to know

Congestive heart failure can cause shortness of breath when reduced heart pumping allows fluid to build up in the lungs. Breathlessness may appear during activity, when lying flat, or at night. A change from your usual breathing pattern deserves clinical attention because infection, rhythm problems, and other lung conditions can feel similar.

Heart failure may raise pressure in the blood vessels around the lungs. Fluid then moves into lung tissue, making each breath feel harder and sometimes causing a cough or wheeze. You may need extra pillows, wake suddenly short of breath, or tire during familiar tasks. Heart rhythm conditions such as Atrial Fibrillation and high blood pressure can contribute to heart strain. Breathlessness alone cannot confirm the cause.

Arrange a clinical review if breathlessness is new, increasing, or limiting walking, sleep, or conversation. Note when it starts, whether lying down worsens it, any recent weight change, ankle swelling, cough, palpitations, and the medicines you take. Seek emergency help for severe breathlessness at rest, inability to speak in full sentences, blue or grey lips, fainting, new confusion, chest pressure, or rapidly worsening symptoms.
