What should you know about complicated cellulitis?

Cellulitis develops when bacteria enter through a break in the skin and infect deeper layers. It often causes expanding redness, warmth, tenderness and swelling. The infection is considered more concerning when the affected area is extensive, symptoms are worsening quickly, deeper structures may be involved, or the person has a condition that makes infection harder to control, such as diabetes or a weakened immune system.

A foot wound, cracked skin or Athlete's Foot can provide an entry point for bacteria. Complicated cellulitis can also cause fever, chills, marked weakness or fast heart rate. These whole-body symptoms can signal that the infection is affecting more than the skin.

Arrange same-day medical evaluation if redness, pain or swelling is spreading, or if you feel unwell. A clinician will examine the skin, look for an entry wound or abscess, and consider whether blood tests, imaging or a hospital assessment is needed. Treatment commonly involves antibiotics; the route and setting depend on the severity, the suspected bacteria and your general health. An abscess may need drainage rather than antibiotics alone.

Keep the area clean and avoid squeezing, puncturing or massaging it. Rest the affected limb and elevate it when practical. Marking the edge of redness can help show whether it is extending, but it does not replace an examination. Take prescribed medicine exactly as directed and report worsening symptoms or a new rash promptly.

Seek emergency care for confusion, fainting, difficulty breathing, severe weakness, rapidly worsening pain, blistering, skin that turns dusky or black, or redness that is spreading quickly. Fever with shaking chills, vomiting or a racing heartbeat also deserves urgent attention. These signs can occur with a serious infection, including Sepsis, and should not be watched at home.

Do not wait for a routine appointment if cellulitis affects the face or is near an eye, if you have diabetes, or if symptoms are not improving after treatment begins. Pain that seems much worse than the skin appearance can indicate deeper tissue involvement and needs immediate assessment.

Ask what may have allowed bacteria to enter, whether an abscess or deeper infection is suspected, and how quickly the redness should stop advancing. You can also ask whether you need tests, intravenous antibiotics or review for an underlying problem such as Type 2 Diabetes.
