The combined pill while breastfeeding
Short answer
The combined pill is usually delayed while breastfeeding is being established because its oestrogen may reduce milk supply and early postnatal clot risk is higher.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Hormone of concern
- Oestrogen can reduce milk production for some people.
- Early postnatal issue
- Temporary clot risk affects when an oestrogen-containing method can begin.
- Alternatives
- Several progestogen-only and non-hormonal methods can be used while breastfeeding.
Can I use the combined pill while breastfeeding?
It may become an option later if feeding is established and you have no medical reason to avoid oestrogen. Your postnatal clinician should advise when it is suitable for your circumstances rather than using a start date chosen from another person's experience.
Milk supply and maternal health both matter
Oestrogen can lower milk production in some breastfeeding parents. A change may show as fewer audible swallows, breasts feeling less full, shorter feeds without satisfaction or concerns about the baby's growth, although these signs can have other causes.
Separately, the body remains more prone to blood clots for a period after delivery. Delivery method, mobility, smoking, migraine with aura, high blood pressure and personal or family clot history influence the assessment.
Protect feeding and contraception
Discuss contraception before relying on breastfeeding alone. Fertility may return before bleeding does, and breastfeeding only prevents pregnancy under specific feeding and menstrual circumstances that can change quickly.
A progestogen-only pill, implant, injection, intrauterine method or condoms may be considered without the oestrogen-related supply concern. If you later begin the combined pill, monitor feeding and wet nappies, attend growth checks, and seek feeding support if supply appears to fall.
Urgent postnatal and clot symptoms
Seek emergency help for sudden shortness of breath, chest pain, coughing blood, collapse, sudden weakness or speech difficulty. New one-sided leg swelling, warmth or pain also needs urgent assessment for a possible clot.
Contact your maternity or baby's healthcare team promptly if the baby is unusually sleepy and difficult to feed, has noticeably fewer wet nappies, or there are concerns about growth. Fever with a painful red area of breast also merits timely medical advice.
Questions for a shared decision
Ask how your delivery and health history affect timing, which methods are least likely to disrupt your feeding goals, and what signs should trigger a milk-supply review. Confirm how to start the chosen method without leaving a contraceptive gap.

Questions people ask
Does breastfeeding itself prevent pregnancy?
It can suppress ovulation only under specific feeding conditions. As feeds change or periods return, protection changes, so discuss a dependable method with a clinician.
What if my milk supply drops after starting the pill?
Keep feeding or expressing regularly and contact a clinician or lactation professional. They can assess feeding, the baby's intake and whether changing contraception is appropriate.
Do combined-pill hormones enter breast milk?
Small amounts of hormones may pass into milk. The practical prescribing concerns are the possible effect on supply and whether oestrogen is safe for the breastfeeding parent's postnatal health.
