How is colon cancer that has spread to the liver treated?

The sequence depends on where disease is present, whether liver deposits can be removed safely, tumour biology, previous treatment, liver function and the person's general health and priorities.

A liver metastasis is made of colorectal cancer cells that travelled from the original bowel tumour. It is treated as metastatic colorectal cancer, not as a new cancer that began in the liver.

Specialists review scans to map deposits within the liver and check the bowel and other body areas. They also consider whether enough working liver would remain after an operation. Molecular testing of tumour tissue can help identify medicines suited to particular tumour features.

A multidisciplinary team may include medical oncology, liver and bowel surgeons, radiology, radiation oncology and specialist nursing. Drug treatment can shrink or control cancer throughout the body; it may be given before a procedure, afterwards, or as the main treatment.

Liver resection may be considered when all visible liver disease can be removed while preserving adequate liver function. Selected deposits may instead be treated with heat-based ablation or focused radiotherapy. The bowel tumour may need surgery or other management as part of the same overall strategy. Plans can change after treatment response is reviewed on imaging.

During treatment, promptly report fever, shaking chills, uncontrolled vomiting, new yellowing of the eyes or skin, increasing abdominal swelling, dark urine, confusion, or pain that is severe or rapidly worsening. The team can assess infection, dehydration, bile-flow problems, treatment toxicity or other complications.

Call emergency services for difficulty breathing, collapse, new severe chest pain or bleeding that will not stop. Use the oncology team's after-hours instructions when symptoms arise outside clinic hours.

Ask what the goal of each proposed treatment is, whether a liver surgeon has reviewed the scans, and what would make surgery or a local procedure feasible.
