Collarbone stress fracture: signs, assessment and recovery

How the structures involved in Stress Fracture differ from normal

Short answer

A collarbone stress fracture is a small bone injury from repeated loading and needs clinical assessment, protection from aggravating activity and a guided return to exercise.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Typical pattern
Focal collarbone pain linked to repeated shoulder loading
First step
Pause aggravating activity and arrange a clinical assessment
Return to activity
Increase load gradually with clinical guidance

What the injury means

A stress fracture develops when repeated strain exceeds the bone's ability to repair itself. In the collarbone, pain may be focused over one spot and become more noticeable with lifting, carrying, pushing or overhead sport. Tenderness or mild swelling can occur. Because collarbone pain can also come from a joint, muscle or an acute break, an examination is needed rather than relying on symptoms alone. A clinician may consider imaging and factors that affect bone health or training load.

What to do now

Stop the movement or sport that triggers the pain and arrange an assessment. Avoid repeatedly testing the sore area, heavy shoulder loading and returning through worsening pain. A clinician can advise whether a sling is useful for comfort and when gentle shoulder movement is appropriate. Recovery planning should address sudden training changes, technique, nutrition and possible contributors such as vitamin D deficiency. Resume loading in stages only after everyday movement is comfortable and the treating clinician agrees the bone is ready.

When to seek care quickly

Seek urgent care after a significant impact, if the collarbone looks deformed, or if bone appears through the skin. New breathing difficulty, chest pain, a rapidly enlarging swelling, or an arm that becomes pale, blue, cold, weak or numb also needs urgent assessment. Otherwise, book a prompt review when focal collarbone pain persists, recurs whenever activity resumes, wakes you at night or interferes with normal arm use.

Questions for your clinician

Ask which movements need temporary restriction, whether imaging is appropriate, and what signs will show that healing is progressing. It is also useful to discuss whether your recent activity pattern, medicines, diet or bone health warrants further evaluation, and what milestones should guide your return to work, resistance training or sport.

Questions people ask

Can I keep training with collarbone pain?

Do not continue activities that reproduce focal collarbone pain until the cause has been assessed. Training through pain can keep loading an injured area and delay recovery.

Will a collarbone stress fracture need surgery?

Management depends on the fracture pattern, symptoms and healing. Many stress injuries are treated by protecting the bone and modifying activity, while a specialist determines whether any procedure is needed.

Why might bone health be checked?

A clinician may look for factors that reduce bone strength or recovery, particularly when the injury followed modest loading or stress injuries have happened before.
