What should you know about coli mastitis?

A clinician will consider whether inflammation is related to breastfeeding, milk stasis, a bacterial infection, or another breast problem. The name of a suspected bacterium does not by itself show how severe the illness is or which medicine will work.

What the term means

Coliform bacteria are a group that includes Escherichia coli. If infection is suspected, a milk, nipple-discharge, or abscess sample may sometimes be cultured to identify the organism and guide antibiotic choice, particularly when illness is severe, recurrent, or not improving. Breast warmth, swelling, redness, tenderness, a firm area, fever, chills, and fatigue can occur with mastitis, but none of these findings can confirm a coliform cause.

What you can do now

Arrange medical advice if you feel unwell, symptoms are worsening, or home care is not helping. If breastfeeding, feed responsively and avoid forceful massage or repeatedly trying to empty the breast beyond your baby's needs; gentle handling and a cool compress may ease swelling. Rest, drink to thirst, and use a pain reliever that your clinician or pharmacist confirms is suitable for you. A lactation professional can check positioning, attachment, nipple injury, and oversupply.

Take any prescribed antibiotic exactly as directed. If a collection of pus is suspected, examination and imaging may be needed because an abscess can require drainage as well as medicine.

When to seek urgent care

Seek urgent assessment for rapidly spreading redness, severe or escalating pain, marked weakness, confusion, faintness, breathing difficulty, or an inability to keep fluids down. Prompt review is also needed for a growing soft or fluid-filled lump, pus, skin breakdown, or fever with worsening breast symptoms. Contact your clinician again if prescribed treatment is not producing clear improvement, since the diagnosis, antibiotic choice, or need for drainage may need reassessment.

Ask whether a culture is appropriate, what changes should trigger reassessment, and whether the examination suggests an abscess. If you are breastfeeding, ask how to maintain comfortable milk removal without increasing inflammation and whether the proposed medicine is compatible with feeding.
