Other presentations include shoulder-tip pain, dizziness, fainting, rectal pressure, or discomfort when passing urine or opening the bowels. Symptoms can develop after a missed period or a positive pregnancy test, and sudden severe pain with weakness can signal internal bleeding.

In an ectopic pregnancy, a fertilised egg implants outside the main cavity of the uterus, most commonly in a fallopian tube. As the pregnancy grows, it can stretch the tube and cause pelvic or abdominal pain. Bleeding from the vagina may look lighter, darker, or different from a normal period.

Symptoms alone cannot confirm the diagnosis. Assessment commonly combines pregnancy testing, blood tests that track pregnancy hormone levels, and transvaginal ultrasound to locate the pregnancy. Pelvic inflammatory disease can damage the fallopian tubes and is one factor clinicians consider when reviewing risk.

Contact a maternity assessment service, gynaecology service, or emergency department promptly if you could be pregnant and have pelvic pain or unusual bleeding. Tell the clinician the date of your last period, any pregnancy-test result, where the pain is located, and whether it is worsening.

Until assessed, avoid driving yourself if you feel faint or unwell. Depending on findings, care may involve close monitoring, medicine, or surgery such as salpingectomy. The appropriate option depends on your symptoms, scan and blood-test results, and overall stability.

Call emergency services now for sudden or severe abdominal or pelvic pain, shoulder-tip pain, fainting, marked dizziness, confusion, breathlessness, pale or clammy skin, or heavy bleeding. These signs can occur with rupture and internal bleeding, which requires immediate treatment.

Do not wait for a routine appointment or a repeat home pregnancy test when these warning signs are present. Emergency assessment is appropriate even if pregnancy has not yet been confirmed.

Ask whether the pregnancy has been located on ultrasound, whether hormone blood tests need repeating, and which change in pain or bleeding should bring you back immediately. If treatment is advised, ask how each option may affect the fallopian tube, recovery, follow-up testing, and plans for a future pregnancy.
