The short answer

A fracture after a minor fall or ordinary movement may be the first clue. Vertebral fractures can cause sudden or persistent back pain, loss of height, or a more stooped posture; hip and wrist fractures are other important presentations.

How the features develop

Bone loses internal strength without producing a sensation that a person can feel. Pain usually reflects a fracture or another musculoskeletal problem rather than the bone-density loss itself. Several small spinal compression fractures may develop quietly and gradually alter height or posture.

Symptoms alone cannot confirm osteoporosis. A clinician considers fracture history, medicines such as long-term corticosteroids, menopause and hormone history, nutrition, mobility, falls, and other illnesses that affect bone. A bone density scan helps estimate bone strength and fracture risk.

Practical next steps

Arrange a clinical review after a low-impact fracture, unexplained height loss, or new persistent back pain. Bring a medicine list and details of earlier fractures or parental hip fractures. Do not start high-dose calcium or vitamin D on your own; the appropriate amount depends on diet, test results, kidney health, and current treatment.

Care may combine weight-bearing and strengthening activity, balance work, adequate nutrition, fall prevention, and bone-strengthening medicine. The plan should match mobility, fracture risk, and other health conditions.

When to seek urgent care

Get urgent assessment after a fall if you cannot stand or bear weight, the hip or limb looks deformed, or pain is severe. New back pain with leg weakness, numbness around the groin, or loss of bladder or bowel control also needs emergency evaluation. Sudden back pain without these warning signs still deserves prompt review when osteoporosis or a spinal fracture is possible.

Can osteoporosis cause pain without a fracture?
