Clinical features of ectopic pregnancy

Pain at the tip of the shoulder, sudden severe abdominal pain, marked dizziness, weakness or fainting can signal internal bleeding and require emergency care.

An ectopic pregnancy develops outside the main cavity of the uterus, most often in a fallopian tube. As it grows, it can stretch or rupture surrounding tissue. Bleeding may look lighter, darker or otherwise different from a usual period, while discomfort can be persistent or come and go.

Symptoms alone cannot confirm the diagnosis because miscarriage, ovarian problems and other causes of pelvic pain can feel similar. Clinicians interpret the symptom pattern alongside a pregnancy test, examination, blood tests that track pregnancy hormone levels and transvaginal ultrasound findings.

If you could be pregnant and develop pelvic or lower abdominal pain, unusual vaginal bleeding, rectal pressure or discomfort when passing urine or stool, arrange prompt medical assessment. Mention the date of your last period, any positive pregnancy test, previous ectopic pregnancy, fertility treatment, tubal surgery or pelvic inflammatory disease.

The first scan or hormone result may not settle the diagnosis when a pregnancy is very early. Your care team may arrange repeat blood tests or imaging and will explain when to return. Keep those follow-up appointments even if pain or bleeding eases.

Go to an emergency department or call local emergency services for sudden intense abdominal or pelvic pain, shoulder-tip pain, fainting, severe dizziness, confusion, clammy skin, difficulty breathing or heavy bleeding. Do not drive yourself if you feel faint or weak. These signs can accompany significant internal bleeding, including a ruptured ectopic pregnancy.

Ask what the scan showed, whether pregnancy hormone testing needs to be repeated, which changes should bring you back immediately, and how the available options could affect the fallopian tube and future pregnancies. If treatment is recommended, ask how follow-up will confirm that it has worked.
