How symptoms develop

When the heart muscle relaxes less easily, pressure can rise as blood enters the chamber. This may make breathing harder during activity or when lying flat. Fluid can collect in the lungs or tissues, leading to a cough, rapid weight change, or swollen ankles. Reduced ability to increase blood flow during activity may leave you unusually tired.

A fluttering or racing sensation can occur when the rhythm is disturbed, although palpitations have many possible causes. Symptoms alone cannot confirm heart failure. A clinician may combine an examination with rhythm checks, blood tests, and heart imaging to identify the cause.

Tracking and assessment

Write down when breathlessness appears, whether you wake short of breath, how far you can walk, and whether swelling changes during the day. Bring this record and a list of medicines to an appointment. Seek assessment for symptoms that are new, persistent, or gradually limiting ordinary tasks.

Your clinician may look for high blood pressure, an irregular rhythm, valve or muscle changes, and other explanations for fatigue or cough. Follow the agreed treatment plan and ask before changing medicines, salt intake, or activity.

Urgent warning signs

Call for urgent help if breathing becomes severe, happens at rest, or starts suddenly; if you have chest pain, fainting, confusion, or blue or grey lips; or if palpitations come with collapse, marked weakness, or breathlessness.

Arrange prompt medical advice when ankle swelling is clearly increasing, you cannot lie flat comfortably, or routine activity becomes noticeably harder. These changes may reflect worsening fluid pressure and should not be judged by symptoms alone.
