Why activity matters

Exercise increases the heart’s need for oxygen, and angina can cause pressure or tightness with walking, climbing, or other effort. Asthma may cause chest constriction, cough, or wheeze during or after activity, particularly in cold or dry air. COPD can also limit breathing during exertion. A strained chest muscle may follow lifting or unfamiliar exercise and may be tender or affected by movement. The trigger is useful information, but it cannot establish which system is responsible.

Pause and arrange assessment

Stop the activity when tightness begins and rest. Do not deliberately repeat the exertion to see whether it returns. Write down the activity, intensity, onset, duration, recovery, and accompanying breathlessness, dizziness, nausea, or sweating. Arrange prompt medical review, particularly if this is new or your exercise tolerance has changed. Use prescribed reliever medicine only according to your personal plan; do not delay assessment to see whether it works.

When it is an emergency

Call emergency services if tightness is severe, lasts after stopping, or is accompanied by fainting, severe breathlessness, cold sweating, nausea, or discomfort spreading to the arm, jaw, back, or shoulder. Urgent help is also needed if symptoms appear at rest or are worsening. Keep activity light until a clinician has advised you, because exertional symptoms should not be self-tested.
