Chemotherapy throat pain: causes, relief and warning signs

Tell your oncology team when the soreness starts, especially if it is limiting food, fluids or medicines. They can look for mouth ulcers, oral thrush, dehydration and other treatable causes.

Chemotherapy may irritate rapidly renewing cells that line the mouth and throat. This inflammation, called mucositis, can cause rawness, burning, ulcers or pain on swallowing. A dry mouth can add friction, while vomiting or acid reflux may further irritate the tissues.

Chemotherapy can also reduce infection-fighting blood cells. White patches, a coated tongue, fever or worsening pain may point to an infection that needs assessment rather than home treatment alone.

Choose soft, moist foods and cool or room-temperature drinks if they feel soothing. Avoid sharp, very hot, spicy or acidic foods when they sting. Small frequent sips, sauces and blended meals can make swallowing easier.

Use a soft toothbrush and follow the mouth-rinse plan supplied by your cancer team. Do not start medicated mouthwashes, lozenges or pain medicines without checking for interactions or ingredients that may irritate damaged tissue. Record when pain occurs and whether swallowing liquids, solids or tablets is affected.

Use the urgent contact number provided by your oncology service if you have a fever, shaking chills, new white patches, rapidly worsening throat pain, or cannot drink enough to stay hydrated. Follow your team's fever threshold because chemotherapy plans differ.

Seek emergency help for trouble breathing, noisy breathing, marked throat or tongue swelling, confusion, fainting, or an inability to swallow saliva. Do not wait for the next clinic appointment when these signs are present.

Ask whether your mouth and throat need examination for mucositis, thrush or another infection; which rinse and pain relief fit your medicines; and how much fluid your team wants you to aim for. Also ask whom to call outside clinic hours if swallowing deteriorates.
