Chemotherapy and microbiology

Some chemotherapy medicines temporarily lower infection-fighting white blood cells or damage protective surfaces in the mouth and gut. If infection is suspected, clinicians may send blood, urine, sputum, wound or line samples to a microbiology laboratory. The laboratory looks for bacteria, fungi or other microorganisms and may test which antimicrobial medicines are likely to work. Treatment may begin before results return when illness could progress quickly, then be refined using the result and the person's condition.

Use the temperature threshold and contact instructions supplied by the oncology team. Wash hands regularly, care for the mouth and any central line as taught, and avoid close contact with people who are acutely unwell. Do not start leftover antibiotics or delay giving a requested sample, because either can make the cause harder to identify. Tell clinicians about recent antibiotics, travel, animal exposure, diarrhoea and any implanted device; these details help them select useful tests.

Contact the oncology service immediately for a fever or sudden illness during chemotherapy, following the team's own temperature guidance. Urgent assessment is also needed for shaking chills, breathing difficulty, confusion, severe weakness, persistent vomiting, rapidly worsening diarrhoea, very little urine, or redness, pain or discharge around a central line. Infection can occur without a marked fever when immune responses are suppressed, so a clear change from normal matters.

Ask when your blood counts are expected to be lowest, which symptoms trigger a call, and where to go outside clinic hours. If samples are taken, ask what was tested, whether a microorganism was found, and whether the antimicrobial plan changed. Check how prescribed antibiotics or antifungals should be timed alongside cancer medicines.
