Chemical pregnancy with no period: what it can mean

A chemical pregnancy is a very early pregnancy loss identified by a positive pregnancy test before a pregnancy is visible on ultrasound. Bleeding often follows, although its timing and flow can differ from a usual period.

A delayed bleed does not confirm that the loss is complete or explain where the pregnancy is. A home test can remain positive while pregnancy hormone levels fall. Alternatively, ovulation may have happened later than assumed, so an ongoing pregnancy may be earlier than expected.

Clinicians may use repeat blood tests for pregnancy hormone and, when appropriate, an ultrasound to distinguish an early ongoing pregnancy from a completed loss or a pregnancy whose location is not yet known.

Contact a maternity service or gynaecology clinician if you had a positive test and have been told or suspect that the pregnancy ended, especially if bleeding has not started. Tell them the dates and results of your tests, any pain, and whether bleeding has occurred at all.

Follow the testing plan you are given rather than relying on symptoms alone. Until the pregnancy location and hormone trend are clear, seek advice before assuming the episode is finished.

Get urgent medical help for severe or worsening lower abdominal pain, pain mainly on one side, shoulder-tip pain, fainting, marked dizziness, weakness, or heavy bleeding. These symptoms need prompt assessment because an ectopic pregnancy can occur even when bleeding is absent or a pregnancy test is becoming fainter.

Also seek timely care for fever, unpleasant-smelling discharge, or pain that is not settling, as these symptoms may indicate infection or another problem requiring examination.

Ask whether you need repeat pregnancy hormone tests, when an ultrasound could be informative, how you will know the pregnancy has resolved, and whom to contact if pain or bleeding begins. If periods remain absent after follow-up is complete, ask when other causes of stopped periods should be assessed.
