Causes of a Small Bald Spot on the Scalp

A small bald spot on the scalp can result from alopecia areata, a fungal infection, hair-pulling or local scalp irritation, so its appearance and associated symptoms help guide assessment. A smooth, clearly outlined patch without scale may fit alopecia areata, while broken hairs, redness, itching, flaking or tenderness can point towards inflammation, infection or physical damage. A clinician may examine the scalp and ask whether the patch appeared suddenly, is enlarging, or is accompanied by changes elsewhere on the body.

Alopecia areata can present with one or more smooth patches where hairs shed from the scalp. Some people notice short tapered hairs around the edge, and the skin may otherwise look unchanged. Scalp ringworm can present with scaling, itch, inflammation, broken hairs or small black dots, and it needs medical treatment because it can spread to other people. Tight hairstyles, extensions, frequent traction, harsh chemical processing or repeated rubbing can also break or pull out hairs. Less commonly, a scar-forming scalp disorder damages follicles and may leave a shiny or altered skin surface; early assessment matters because the treatment approach differs.

A bald patch is a symptom rather than a diagnosis. A photograph taken in good light and a note of recent hair-care practices can help you describe the change accurately, but they cannot replace an examination.

Arrange a dermatology or primary-care appointment if the patch is new, enlarging, recurring or unexplained. Until then, handle the area gently: avoid tight styles, scratching, vigorous brushing and new chemical products on the scalp. Do not share combs, brushes, hats or pillows if there is scaling or a possible infection. Avoid applying steroid, antifungal or hair-growth products without advice, because the wrong product can alter the appearance or delay the right treatment. Take a clear image now and compare it with later images using similar lighting.

Seek prompt medical advice if the scalp is painful, swollen, warm, draining fluid or developing crusts, or if you have fever or tender neck glands. Arrange an earlier review when hair loss is spreading quickly, several patches appear, eyebrows or eyelashes are affected, or the skin becomes smooth and shiny. A child with a scaly patch, broken hairs or significant itch should be assessed because scalp ringworm can require prescription treatment. Sudden extensive shedding or a severe reaction after a product also warrants prompt evaluation.
