Causes of lower abdominal pain after ovulation

Brief one-sided discomfort can occur around ovulation and may linger as pelvic tissues respond to the cycle. Other possibilities include an ovarian cyst, endometriosis, adenomyosis, fibroids, pelvic inflammatory disease, bowel or urinary problems, and early-pregnancy complications.

Timing alone cannot identify the cause. Pain from an ovarian cyst may be focused on one side, while endometriosis or adenomyosis can produce recurring pelvic pain linked with periods. Pelvic inflammatory disease may present with pelvic pain alongside unusual discharge, pain during sex, bleeding between periods, or fever.

Consider whether pregnancy is possible. Ectopic pregnancy can present with one-sided pelvic or abdominal pain, vaginal bleeding, shoulder-tip pain, dizziness, or fainting. It requires urgent assessment because a pregnancy outside the uterus can cause internal bleeding.

Record when the pain began, its exact location, whether it comes and goes, and any bleeding, discharge, urinary symptoms, bowel changes, nausea, or pain during sex. A home pregnancy test is sensible if pregnancy is possible, even when bleeding resembles a period.

For mild discomfort without warning signs, rest, gentle heat, and a pain medicine that is safe for you may help. Check with a pharmacist or clinician before taking medicine if you could be pregnant, have stomach or kidney problems, take blood thinners, or have medicine allergies.

Seek emergency care for sudden severe pain, fainting, marked dizziness, shoulder-tip pain, heavy bleeding, difficulty breathing, or pain with a positive pregnancy test. These features can signal internal bleeding or another urgent problem.

Arrange a prompt appointment if pain persists, repeatedly returns after ovulation, disrupts daily activities, or occurs with fever, vomiting, unusual discharge, painful urination, or bleeding between periods. Examination, pregnancy testing, urine or swab tests, and ultrasound may be considered according to the pattern.

Ask which causes fit the timing and location, whether pregnancy testing or pelvic ultrasound is appropriate, and what change in symptoms should trigger urgent care. Mention previous cysts, endometriosis, pelvic infections, operations, fertility treatment, and all contraception or medicines you use.
