Why swelling can happen
Pregnancy-related fluid does not leave the body instantly, and the legs may remain puffy while circulation readjusts after birth. Spending long periods sitting, standing, or recovering from a caesarean birth can slow the return of blood from the legs. Varicose veins may make swelling more noticeable. Swelling in both legs that gradually improves with movement and elevation is often less concerning than a sudden change on one side. A blood clot, high blood pressure after pregnancy, or problems involving the heart or kidneys can also cause swelling and requires clinical assessment. Obesity can add mechanical pressure but should not be assumed to explain a new or marked change.
Helpful measures
Walk gently as your recovery allows, avoid sitting still for long periods, and raise your legs when resting. Wear comfortable footwear and drink normally unless your maternity team has given different advice. Do not massage a painful or suddenly swollen leg until a clot has been excluded. Arrange a postnatal review if swelling is persistent, worsening, or accompanied by high blood pressure, headache, visual changes, or reduced urine.

Seek emergency help for sudden breathlessness, chest pain, coughing blood, fainting, or severe weakness. Contact urgent medical services for one-sided leg swelling with pain, warmth, redness, or tenderness, or for rapidly increasing swelling. Seek prompt assessment for severe headache, vision changes, upper abdominal pain, or marked swelling of the face and hands after delivery, as these can accompany a post-pregnancy blood-pressure problem.
