Causes of left heart failure

Short answer
Left heart failure can result from long-term high blood pressure, coronary artery disease, heart muscle disease, valve problems, or an abnormal rhythm.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Common contributors
- Hypertension, coronary disease, valve disease, muscle disease, or rhythm problems
- Lung-related signs
- Breathlessness or cough can reflect pressure backing up toward the lungs
How the left side is affected
The left side of the heart receives oxygen-rich blood and pumps it around the body. Hypertension can make the heart work against greater resistance, causing the muscle to stiffen or weaken over time. A blocked or narrowed coronary artery can damage heart muscle. Atrial fibrillation and other rhythm disturbances may reduce efficient filling and pumping. Valve disease, inflammation, inherited heart muscle conditions, and previous heart injury are other possible contributors. When pressure backs up toward the lungs, a person may develop breathlessness, a cough, reduced exercise tolerance, or fatigue. Swollen ankles can occur when heart failure affects fluid balance more broadly.
What to do next
Arrange a medical review if breathlessness, fatigue, palpitations, or ankle swelling is new, persistent, or worsening. Assessment may include blood pressure measurement, an examination, an electrocardiogram, blood tests, and heart imaging. Keep a record of symptoms, their triggers, and whether they are worse when lying down. Take prescribed medicines consistently, and do not change doses or stop treatment without advice. Managing hypertension, rhythm problems, and other underlying conditions can reduce strain on the heart.
When to seek urgent help
Call emergency services for severe or sudden breathlessness, chest pressure, fainting, confusion, blue or grey lips, or a rapid deterioration. Seek prompt clinical advice if you wake gasping, cannot lie flat because of breathing difficulty, or notice a rapid increase in swelling or body weight. These patterns can signal worsening fluid congestion and need timely assessment.
Questions for your doctor
Ask whether the problem is mainly reduced pumping, impaired filling, or both; which cause is suspected; and which tests will clarify it. Discuss blood-pressure goals, rhythm treatment, fluid guidance, and the timing of a chronic disease review.

Questions people ask
Can hypertension cause left heart failure?
Yes. Long-term high blood pressure can make the left heart work harder and eventually become stiff or weak.
Are palpitations a sign of left heart failure?
Palpitations may reflect an abnormal rhythm that contributes to heart strain, but they also have other causes and need assessment.
