What causes gout in the feet?

The big-toe joint is a familiar site, but gout can also affect the midfoot or ankle. An attack may follow dehydration, illness, heavy alcohol intake, a large meal rich in purines, or a change in certain medicines, although no obvious trigger is always found.

How urate builds up

Urate is produced when the body breaks down purines. Levels can rise when the kidneys remove too little urate, the body produces more than it can clear, or both occur together. Reduced kidney function, obesity, family tendency, some blood conditions, alcohol, and medicines such as certain diuretics can contribute.

Crystal deposits may remain quiet between flares. During a flare, the affected foot joint can become intensely painful, warm, red, swollen, and very sensitive to touch. These features describe inflammation but do not by themselves confirm its cause.

What to do during and after a flare

Rest the foot, raise it, avoid pressure from tight footwear, and use a wrapped cold pack for comfort. Contact a clinician early because anti-inflammatory treatment works more effectively when started promptly and must be chosen around kidney health, stomach problems, blood-thinning medicines, and other conditions.

Do not stop a prescribed long-term urate-lowering medicine during a flare unless your prescriber tells you to. After symptoms settle, review hydration, alcohol, medicines, kidney health, and whether repeated attacks or crystal deposits make preventive treatment appropriate.

When foot swelling needs urgent care

Seek same-day medical assessment for a first episode of a hot, red, severely painful joint, especially with fever, chills, broken skin, a wound, or feeling generally unwell. A joint infection can resemble gout and requires urgent treatment.

Get urgent help after an injury if you cannot bear weight, the foot looks deformed, or it becomes pale, blue, cold, or numb. Arrange a routine review if attacks recur, several joints become involved, or firm lumps develop near joints.

Can footwear cause gout in the feet?

Footwear does not create urate crystals. Tight shoes or minor pressure may aggravate an already inflamed joint and make a flare feel worse.

Does food alone cause foot gout?

Diet can influence urate and may help trigger a flare, but kidney clearance, inherited tendency, medicines, alcohol, body weight, and other health conditions can also contribute.

Why can gout return in the same foot?

Crystals may remain in joint tissues between attacks. Unless the urate level is controlled over time, inflammation can return at the same or another joint.
