Causes of blood clots in sputum

Short answer
Blood clots in sputum can result from forceful coughing, airway infection, bronchiectasis, COPD, or pneumonia, but they need prompt medical assessment.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

- Breathlessness, chest pain, faintness, or repeated clots
What the appearance may tell you
A clot can form when bleeding from a small irritated airway mixes with mucus. Repeated hard coughing may injure delicate lining, while Acute Bronchitis or Pneumonia can inflame it. Bronchiectasis can leave widened airways that collect mucus and bleed intermittently. COPD may make airway irritation and infection more likely.
Blood from the nose, mouth, or Chronic Sinusitis can run backwards and be mistaken for lung sputum. Bright red blood, dark clots, or rust-coloured mucus do not identify the cause by themselves. The amount, recurrence, breathlessness, fever, chest pain, smoking history, and medicines that affect bleeding all matter.
What to do now
Arrange prompt medical review, particularly if this is new or recurrent. Note the colour and approximate amount, whether it followed coughing, and any fever, wheeze, chest pain, or breathlessness. If possible, take a photograph for the clinician. Do not suppress a productive cough with leftover medicines, and do not start Antibiotic Treatment without an assessment; antibiotics help only some causes.
When to seek emergency help
Call emergency services for more than a small streak, repeated clots, difficulty breathing, chest pain, faintness, blue lips, confusion, or bleeding that will not stop. Seek urgent care with high fever, severe weakness, or a sudden decline. Sit upright, avoid exertion, and do not drive if you feel faint or breathless.
Questions for your doctor
Ask whether you need a chest examination, oxygen check, sputum testing, imaging, or blood tests. Tell the clinician about smoking or vaping, lung disease, recent infections, nosebleeds, and every medicine or supplement that may affect bleeding.

Questions people ask
Can forceful coughing cause a blood clot in sputum?
It can irritate small airway blood vessels and produce blood mixed with mucus, but a clot should still be assessed, especially when it recurs.
Should I take antibiotics for blood in sputum?
Not without medical advice. Antibiotics treat selected bacterial infections and are not suitable for every cause of bleeding.
