What it may mean

A sleeping seizure can involve rhythmic jerking, stiff muscles, staring, unusual sounds, loss of bladder control, or movements that stop without the person responding. Possible triggers or causes include low blood sugar, alcohol withdrawal, infection affecting the brain, medication changes, head injury, or epilepsy. A seizure can happen without a known cause. A single episode does not by itself establish epilepsy.

What to do during and after it

Keep the person away from stairs, sharp objects, and hard edges. Cushion the head, loosen tight clothing around the neck, and turn them onto their side when the movements ease. Time the episode from its start. Do not restrain them, put anything in their mouth, or give food, drink, or medicine until they are fully alert.

Stay nearby during recovery and speak calmly. Note what the body did, whether breathing changed, any injury, fever, recent illness, alcohol or medicine changes, and how long confusion lasted. Arrange medical advice after a first suspected seizure even if the person seems well afterward.

When to seek urgent care

Call emergency services if the seizure lasts five minutes or longer, another seizure starts before full recovery, breathing remains abnormal, the person is seriously injured, the event occurred in water, or they are pregnant or have diabetes. Seek urgent assessment for a first seizure, a new pattern, fever with severe headache or stiff neck, or persistent weakness or confusion.
