Cast for a broken back: what to know

Treatment depends on which vertebra is fractured, whether its alignment is stable, and whether nerves or the spinal cord are affected. Some stable fractures can heal with activity limits and a brace; unstable injuries may need surgical stabilisation.

A rigid spinal orthosis controls selected trunk movements and supports alignment while bone heals. It is shaped and adjusted for the injured level; a limb cast cannot provide the same controlled support. The brace also acts as a reminder to avoid bending, twisting or lifting beyond the care plan.

Not every vertebral fracture needs bracing. The specialist may use examination and imaging to determine stability and to check healing before restrictions are relaxed.

Wear the brace in the situations specified by the treating team and use their taught method for putting it on and removing it. Keep a smooth, dry layer between the brace and skin if advised, inspect pressure areas daily, and report rubbing that persists. Do not alter straps, pads or rigid sections without guidance.

Use safe transfer techniques and follow restrictions on driving, work, lifting and exercise. Walking may be encouraged for some stable injuries, but activity should match the individual plan rather than pain tolerance alone.

Call emergency services for new leg weakness, spreading numbness, numbness around the groin or buttocks, loss of bladder or bowel control, or difficulty breathing after a spinal injury. Keep movement to a minimum while awaiting help unless the surroundings are dangerous.

Contact the treating service promptly if back pain suddenly worsens, the brace breaks or no longer fits, or the skin develops an open sore. Fever with increasing spinal pain also warrants urgent clinical assessment.

Ask whether the fracture is stable, exactly when the brace must be worn, which sleeping and washing methods are safe, and what symptoms change the urgency. Confirm how imaging will guide brace removal and when supervised rehabilitation can begin.
