What should you know about carcinoid bone metastases?

Carcinoid tumours belong to the broader group of neuroendocrine tumours. Spread to bone means the cancer is metastatic; it is not a new bone cancer. Management aims to control the underlying tumour, protect affected bones, relieve pain and prevent complications such as fracture or pressure on the spinal cord.

Imaging helps show which bones are involved, whether a weight-bearing bone is weakened and whether disease lies near the spine or nerves. Blood tests may check calcium levels, kidney function and other markers relevant to treatment. A biopsy is sometimes needed if the finding is uncertain or its behaviour differs from known disease.

Treatment is tailored to the tumour and the affected site. Options may include systemic treatment for the neuroendocrine tumour, radiotherapy to ease local pain, bone-strengthening medicine, pain relief, or an orthopaedic procedure to support a bone at risk of breaking. More than one approach may be used.

Report new focal bone pain instead of waiting for the next routine visit, especially pain in the hip, thigh, back or upper arm. Until the team assesses a painful weight-bearing area, avoid heavy lifting and high-impact activity. A physiotherapist can advise on safer movement, walking aids and exercises suited to the strength of the bone.

Take pain medicines on the prescribed schedule and describe breakthrough pain, sleep disruption and activities that have become difficult. If bone-strengthening treatment is planned, tell the team about dental problems and arrange the dental checks they recommend before starting.

Go to an emergency department for new leg weakness, numbness around the buttocks or genitals, difficulty walking, or loss of bladder or bowel control. With cancer affecting bone, these symptoms can indicate spinal cord or nerve compression and treatment should not wait.

Sudden severe pain, inability to put weight on a limb, or a limb that looks shortened or misshapen may signal a fracture. New confusion, pronounced thirst, repeated vomiting or unusual drowsiness also needs urgent assessment because altered calcium levels can cause these symptoms.

Ask which bones are affected, whether any area is at risk of fracture, what activity is safe and whether a surgical or radiotherapy opinion is needed. Discuss the aim of each treatment, dental precautions for bone-strengthening medicine, the pain plan and which symptoms should trigger the emergency route.
