Can polycystic ovary syndrome cause anovulation?

Ovulation may occur unpredictably rather than stopping completely, so irregular bleeding does not reliably show whether or when an egg was released.

In PCOS, follicles may begin developing but fail to mature and release an egg in a regular pattern. Altered androgen and insulin activity can contribute to this interruption in ovarian signalling.

Possible clues include widely spaced or absent periods, unpredictable bleeding, acne or increased facial or body hair. These features can support an assessment, but none confirms ovulation status by itself.

A clinician may review your cycle record, symptoms and medicines, exclude pregnancy, and select hormone tests or an ultrasound when appropriate. Other causes of missed ovulation, including thyroid or prolactin disorders, may need checking.

Management depends on your goal. If pregnancy is not planned, treatment may regulate bleeding and protect the uterine lining. If you want to conceive, medicines that encourage ovulation may be considered. Nutrition, movement, sleep and gradual weight management can support metabolic health where relevant.

Make an appointment if periods are repeatedly absent or far apart, bleeding is difficult to predict, or you are trying to conceive and cycles appear irregular. Long gaps without a bleed can allow the uterine lining to build up, so do not leave the pattern unassessed.

Get urgent help for very heavy bleeding with faintness or breathlessness, or for a positive pregnancy test accompanied by one-sided pelvic pain, shoulder-tip pain, dizziness or collapse.
