Why the sensations can overlap
Both sensations may involve a dull ache or tightening low in the abdomen. With Braxton Hicks, you may also feel the bump firm and then release, without a steadily developing rhythm. The feeling can vary from one pregnancy to another, so the pattern and accompanying changes carry more meaning than whether it resembles a period.
Labour can begin with period-like cramps and may later form stronger, closer contractions. Preterm labour can also present with cramping, pelvic pressure, low backache, or changed discharge. These signs call for assessment when they persist or occur before full term.
What to check at home
Note whether your whole abdomen firms, when each episode begins, and whether the cramps are becoming more frequent or intense. If you have no warning signs, pause activity, drink water, empty your bladder, and change position. Notice whether these measures alter the sensation.
Keep monitoring your baby's usual movements. When calling maternity care, report your pregnancy stage, the cramp pattern, back or pelvic pressure, discharge, possible fluid leakage, bleeding, and movement changes. Do not take medicine for new pregnancy pain without checking what is suitable for you.
When cramping needs urgent attention
Contact your maternity unit urgently if cramps are regular, worsening, or persistent; if they occur before full term; or if you have vaginal bleeding, a gush or trickle of fluid, reduced fetal movement, severe pain, fever, faintness, or marked pelvic pressure. Assessment is appropriate even when you are unsure whether the sensation is a contraction.
Get emergency help for heavy bleeding, collapse, unbearable or continuous pain, or signs that birth may be imminent. Avoid travelling alone when your symptoms could worsen on the way.

Ask how your stage of pregnancy changes the threshold for calling, which pain relief is suitable, and where to attend if cramps continue. Explain whether the ache is central or one-sided and whether urinary, bowel, or vaginal symptoms accompany it.
