Brain radiation for lung cancer: planning and care

Focused radiation can treat selected targets with high precision, while whole-brain treatment covers brain tissue more broadly. The care team may coordinate radiation with steroids, systemic medicines, surgery or supportive care.

Lung cancer can spread through the bloodstream and form brain metastases. The number, size and position of deposits, swelling, previous brain treatment and overall cancer status all influence radiation planning. The tumour's molecular profile can also affect which systemic options are available.

Treatment planning uses imaging to shape radiation beams and protect nearby structures as much as possible. A fitted mask may keep the head in a reproducible position. Likely effects depend on the treated area and may include fatigue, scalp irritation, hair loss, headache or nausea; the team can explain which apply to the proposed technique.

Bring a complete medicine list and tell the radiation team about previous brain radiation, seizures, implanted devices and difficulty lying flat or wearing a mask. Take prescribed steroids and anti-seizure treatment on schedule, and request clear instructions before changing either medicine.

Track headaches, nausea, weakness, balance, speech, vision and seizure activity. Ask which skin and hair products are suitable for the treatment area. Confirm who to call after hours and when systemic lung cancer treatment will pause, continue or restart around radiation.

Call emergency services for a first seizure, a seizure followed by poor recovery, sudden one-sided weakness, new inability to speak clearly, collapse, severe confusion, loss of consciousness, or a sudden extreme headache.

Notify the cancer team promptly about worsening headache, repeated vomiting, increasing drowsiness, new balance or vision problems, fever during treatment, or neurological symptoms that worsen as a steroid dose falls. Do not wait for the next radiation appointment to report these changes.

Ask why the recommended radiation field fits the MRI findings, what benefit the team expects, and how side effects will be managed. Discuss possible effects on memory and daily function, whether surgery or systemic therapy also has a role, and when imaging will be repeated to assess the brain.
