Bowel pain during sex

Pain described as being in the bowel may actually arise from the rectum, pelvic floor, uterus, ovaries, vagina, or tissue between these structures. Deep penetration can press on an already sensitive area. Constipation, trapped gas, bowel inflammation, pelvic-floor muscle spasm, and scar tissue are among the possibilities.

Endometriosis can present with deep pain during sex and painful bowel movements, especially when tissue behind the uterus or near the bowel is involved. Pelvic inflammatory disease may present with pelvic pain and pain during sex. These associations describe how conditions can present; the symptom alone does not show which condition is responsible.

Stop any activity that hurts. Try shallower penetration, positions that let you control depth, slower movement, and generous lubricant if dryness or friction contributes. Avoid penetration during a flare and choose other forms of intimacy if preferred; pain does not need to be pushed through.

Record whether pain occurs with deep penetration, orgasm, bowel movements, menstruation, urination, constipation, or particular positions. Note bleeding, discharge, bloating, fever, and changes in stool. A clinician may use this pattern to choose a pelvic examination, infection testing, imaging, bowel evaluation, or referral for pelvic-floor physiotherapy.

Book an appointment when the pain recurs, limits intimacy, accompanies painful bowel movements, or is linked with menstrual or bowel changes. Seek urgent care for sudden severe pelvic or abdominal pain, fainting, marked dizziness, fever, persistent vomiting, heavy vaginal bleeding, blood in stool, a rigid swollen abdomen, or possible pregnancy with significant pain or bleeding.

Ask which pelvic and bowel structures could match the pain location, whether infection testing or imaging would be useful, and whether pelvic-floor muscles seem involved. If symptoms track with periods or bowel movements, ask how endometriosis is evaluated and what symptom relief is reasonable while the cause is being investigated.
