Body swelling after a C-section: what to know

Short answer
Swelling after a C-section can follow pregnancy-related fluid retention, intravenous fluids and reduced movement, but certain patterns need urgent assessment.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Common locations
- Feet, ankles, lower legs and hands
- Useful detail to track
- Whether swelling is equal on both sides and gradually easing
- Emergency pattern
- Breathlessness, chest pain or sudden one-sided painful leg swelling
The short answer
Both feet, ankles, legs, hands or the area around the incision may feel puffy. The overall pattern should gradually settle rather than rapidly spread or become painful.
Why swelling can happen
During pregnancy, the body holds extra fluid. Fluid given through a vein during surgery can add to this, while hormonal shifts and less walking after the operation can slow the return of fluid from the legs. Gravity often makes ankle and foot puffiness more noticeable later in the day.
Mild swelling on both sides without redness or severe pain can fit normal recovery. Swelling around the wound deserves a closer look if it is increasing, hot, red, opening or leaking. Pre-eclampsia can first present after birth and may include sudden swelling alongside a severe headache, visual changes or pain beneath the ribs.
Comfort and recovery steps
Take short, gentle walks as your maternity team has advised, change position regularly and rest with your legs supported. Wear loose clothing and footwear, drink according to thirst, and continue prescribed medicines exactly as directed. Avoid taking water tablets or herbal remedies unless a clinician who knows your postnatal care has approved them.
Notice whether swelling is on both sides, whether pressing leaves a dent, and whether it is improving from day to day. If you have a blood-pressure monitor and were told to use it, record the readings and symptoms for your care team rather than changing treatment yourself.
When to seek medical care
Seek emergency help for difficulty breathing, chest pain, coughing blood, fainting or a suddenly swollen and painful leg, especially when only one side is affected. These symptoms need immediate assessment for a blood clot or another serious complication.
Contact your maternity service promptly for rapidly increasing swelling, a severe or persistent headache, blurred vision or flashing lights, pain under the ribs, vomiting, feeling markedly unwell, or a blood-pressure reading your team has identified as concerning. Fever, worsening wound pain, spreading redness or discharge also needs timely review.
Questions for your doctor
Ask whether the location and symmetry of your swelling fit expected recovery, how often to check your blood pressure, which wound changes require examination, and what level of walking is suitable for you. Mention any headache, vision change, breathlessness, calf pain, previous high blood pressure, or medicines you are taking.

Questions people ask
Can intravenous fluids make me look more swollen after a C-section?
Yes. Fluids given around surgery can temporarily add to fluid already retained during pregnancy. Movement and normal fluid regulation usually help the body clear it during recovery.
Is swelling in only one leg different from swelling in both feet?
Yes. Sudden one-sided swelling with calf or thigh pain, warmth or redness needs urgent assessment for a possible blood clot, particularly if breathlessness or chest pain occurs.
Should I massage a swollen, painful calf?
No. Do not massage a newly swollen painful calf; reduce unnecessary movement and obtain urgent medical assessment.
