Body aches when ovulating: what to know

Ovulation can cause one-sided lower abdominal or pelvic discomfort when an ovary releases an egg. Some people also notice bloating, breast tenderness, mild backache or a change in discharge. These symptoms tend to follow a cycle pattern and settle without becoming progressively worse. Aches throughout the body, fever, vomiting, faintness, painful urination or unusual bleeding are not typical features to dismiss as ovulation. Pelvic pain may also have causes such as an ovarian cyst, infection, endometriosis or pregnancy-related problems, so timing alone cannot identify the cause.

Record the first day of your period, the suspected ovulation timing, pain location, bleeding and associated symptoms. Rest, fluids, gentle movement and a warm pack over the lower abdomen may reduce mild discomfort. Ask a pharmacist or clinician which pain reliever is suitable, particularly if pregnancy is possible, you have stomach or kidney disease, or you take regular medicines. Avoid using cycle timing to delay assessment when the pain is new, intense or different from your usual pattern.

Seek urgent care for severe or rapidly worsening pelvic pain, fainting, shoulder-tip pain, heavy bleeding, fever, repeated vomiting, a rigid abdomen, or a positive pregnancy test with pain or bleeding. Arrange a medical appointment if aches last beyond the expected cycle window, recur with increasing intensity, disturb daily activities, or occur with unusual discharge or pain during sex. A clinician may ask about your cycle and examine you before deciding whether testing or imaging is appropriate.
